CARE PLANS

If you have a chronic or terminal medical condition, you may be eligible for Medicare rebates for up to 5 sessions per calendar year for certain Allied Health services.

 

 

  Examples of chronic medical conditions include Lung disease including Asthma and COPD, Diabetes, Heart disease including coronary artery disease and heart failure, osteoporosis, arthritis, stroke, etc.

All GP Management Plans/Team Care Arrangement appointments are bulk billed. This would typically involve a discussion with our Practice Nurse followed by an appointment with your doctor.  Please speak to your doctor if you would like further information
