CARE PLANS

GP MANAGEMENT PLANS AND TEAM CARE ARRANGEMENTS

If you have a chronic medical condition, you may be eligible for Medicare rebates for up to 5 sessions per calendar year for certain Allied Health services.

 

 Examples of chronic medical conditions include lung disease including asthma and COPD, diabetes, heart disease including coronary artery disease and heart failure, osteoporosis, arthritis, and stroke.

All GP Management Plans/Team Care Arrangement appointments are bulk billed. This will typically involve a discussion with our Practice Nurse followed by an appointment with your doctor.  Please speak to your doctor if you would like further information
